Price is determined on condition of the coat, size and behaviour of your dog.
Please advise your groomer if your dog has behavioural / aggression issues towards other dogs, has allergies or any other concerns we should be aware of (such as raised moles, arthritis, pustules, any recent surgeries, etc.).
Grooming Release Agreement
All medical expenses for veterinary care will be covered by the pet’s owner upon signing this contract/ agreement. Although accidents are very rare, there is a risk when dealing with animals. Grooming equipment is sharp, and although we use extreme caution and care in all situations, possible problems could occur including cuts, scratches and razor burn, etc. In most cases this can happen when an animal is wiggling or moving around. I hereby release ROXY ROVER PET SPAW, its directors, officers, shareholders, employees, assistants, members and agents from any and all liability, claims, suits, actions, loss, injury or damage of any nature or kind, which I, the Dog/Cat and any third party or their pet may suffer, including specifically, but not without limitation, any accidents, injury, damage, loss or death of my animal on or off the premises at #40 – 1935 37th St. SW, Calgary, Alberta whatsoever while in attendance at ROXY ROVER PET SPAW.
I allow ROXY ROVER PET SPAW staff to contact my/a veterinarian as deemed necessary should any injuries require medical attention. I agree that I am responsible for any medical and legal responsibility for my pets’ actions. I approve Veterinary care for my dog up to the amount of $500, if ROXY ROVER PET SPAW cannot reach the owner prior to receiving veterinary care. If care exceeds the amount of $500, veterinary care will be postponed until the owner is contacted.
A valid credit card must be left on file for such instances of veterinary care. I represent that my dog/cat is in good health, is current on all required vaccinations (FVRCP and FVR (for cats), DHLPP (for dogs) and Rabies), is current and has not been ill with any known contagious diseases. I understand that Bordetella (Kennel Cough) and Flea/Tick medication are recommended but not mandatory and understand that these may be contagious amongst groups of dogs. I understand that if my dog/cat is found to have fleas or lice, ROXY ROVER PET SPAW will be required to refuse service and your appointment can be rescheduled. Your dog/cat must be treated for fleas or lice at least 2 times prior to returning for a new appointment. Ticks will be removed by the groomer; there will be an extra charge for tick removal.
I understand that ROXY ROVER PET SPAW has the right to refuse any services at any time. In the event that your pet is too stressed or becomes dangerous to groom, ROXY ROVER PET SPAW has the right to refuse grooming services, stop grooming services, or cancel grooming services at any time before, during, or after grooming, and client will be charged a grooming fee (for what was done up until that point). This is specifically important for puppies, senior cats/dogs or any recent treated medical conditions and surgeries. I understand that ROXY ROVER PET SPAW has the right to muzzle your cat/dog if it acts in a way that is dangerous to staff. Dogs or cats that require muzzling or extra staff to help manage them will be charged at a higher than standard grooming rate. This is because dogs or cats showing aggression or acute nervousness take much longer to groom. There is a higher risk of your animal being cut if it is matted. Therefore, I understand that animals with severely matted coats require extra attention. Mats in an animal’s coat grow tight, and can ultimately damage and tear the animal’s skin, which provides a breeding ground for parasite infestations. Your dog/cat may need to be shaved, and a specific cut may not be able to be done.
I agree that a cancellation fee of $65 will be applied if an appointment is canceled less than 24 hours of the scheduled appointment OR if you are 30 minutes late past the scheduled appointment for your pet. We will not be able to groom your dog if you are more than 30 minutes past your scheduled appointment. A late pickup fee of $10.00/hr. will be charged for every hour you are late, past 1 hour you have been called to pick up your dog/cat.
I agree that my dog/cat may be videotaped, photographed, and/or recorded. ROXY ROVER PET SPAW will be the exclusive owner to the results and all proceeds of such taping, photography, and recording. I certify that I have read and understand the rules and regulations herein and that I have read and understand this agreement. I agree to abide by the rules and regulations and accept all terms, conditions, and statements of this agreement and confirm the truthfulness of the contents of the application form completed by me. I agree that I am of the full age of eighteen years of age.
